In a rural clinic in eastern Rwanda, a community health worker once spent hours deciphering faded handwritten notes before deciding whether to refer a sick child. Records got lost, treatments were delayed, and lives hung in the balance. Today, that same worker opens a mobile app linked to the national system. In seconds, the child’s full history appears, complete with previous visits, test results, and recommended protocols. The difference? Digital Public Infrastructure (DPI).
Across Africa, DPI — the foundational, reusable digital layers of identity, interoperable data exchange, and secure payments — is quietly becoming the most important infrastructure investment in healthcare. While roads, hospitals, and equipment remain essential, they cannot deliver their full value without this “invisible plumbing” that connects every part of the system.
DPI turns fragmented health services into a coordinated network. A patient’s digital identity allows seamless movement between facilities without repeating tests or carrying paper folders. Real-time data platforms feed live dashboards that show disease trends, stock levels, and staffing gaps. Digital payment rails enable instant insurance claims, salary disbursements to health workers, and targeted subsidies. The result is faster care, fewer errors, and dramatically better use of limited resources.
Rwanda is leading the way. At the Africa HealthTech Summit 2025 in Kigali, the country showcased how DPI is powering connected, inclusive healthcare. The Rwanda Information Society Authority (RISA) has established a National Health Intelligence Centre to strengthen data collection and analysis. “You cannot digitize an entire health system overnight. It starts with data collection and integration,” explains RISA CEO Antoine Sebera. Strong data protection laws and data-sharing policies now guide secure interoperability. Initiatives such as Connect Rwanda and the Digital Ambassador Programme are extending access and literacy to remote communities.
Other countries are advancing rapidly too. Kenya’s digital ID systems increasingly link citizens to health services, improving targeting and reducing fraud. Nigeria and Ghana use open platforms like DHIS2 to manage health data at scale, giving policymakers clear visibility into performance. These building blocks allow electronic health records, telemedicine, and supply-chain tracking to operate on shared foundations rather than isolated silos.
The impact on health infrastructure is already measurable. Interoperability cuts wasteful duplication of diagnostics and paperwork. Community health workers equipped with digital tools register patients, submit reports, and receive payments faster — improving retention in hard-to-reach areas. Integrated surveillance systems enable quicker outbreak detection and response. Digital insurance and payment platforms reduce out-of-pocket costs that push families into poverty, directly supporting universal health coverage goals.
Of course, no infrastructure project is without risks. Uganda’s earlier digital ID challenges showed how exclusion can unintentionally block access to health services for vulnerable groups. Privacy breaches, weak connectivity, and low digital literacy can widen inequalities if not addressed. That is why inclusive design, offline functionality, strong governance, and continuous capacity building must sit at the centre of every DPI rollout.
The path forward is clear. DPI is not a luxury add-on — it is the foundational infrastructure that multiplies the value of every hospital, clinic, and health worker. When governments, development partners, and the private sector collaborate on open standards, shared data platforms, and citizen-centric design, Africa can leapfrog legacy limitations and build world-class health systems.
As Sebera rightly observed, “DPI is not something one institution can deliver alone. It is about collaboration, inclusion, and shared progress.
”The continent now has the technology, the lessons, and the momentum. The only question is how boldly and inclusively we choose to build this digital backbone — because the health of millions depends on it.
